That's simple.  Many problems with the body, cancer comes to mind, and
reproductive cysts are certainly no exception, are caused by demons, just
as was (supposedly) widely believed hundreds of years ago.  That's the
demons face in the cyst.

 Or it could be prophetic.  It may be the face of the child that that
ovary either will produce, or will never produce because of the cyst.
