Have you ever run across an RPG called Trauma?  It was a French game from
the '80s; the English translation came via Australia around 1990 or
so.  The basic concept was that the PCs are the players themselves,
coverted to stats, and then thrust into extreme situations much like what
you describe above.  The results are pretty intense, and I'm not sure I'd
recommend playing it with anyone who is, er, easily traumatized.
