I meant to get a book called "Base Instinct" (yeah, the title was  
probably a sop to move a few more copies) in which a neurologist  
(neurobiologist? neuropsychologist?) examined really extreme, extreme  
criminals.  His theory was that the sorts of stresses that could  
produce behavior that far off the norm would fall into three categories.

1) People with physical injuries to the brain.
2) People with family histories of clinical insanity.
3) People who'd undergone abuse as children.

His results surprised him.  IIRC, he could not locate a single mass  
murderer who didn't have at least one of his three traits.  Most had  
at least two.  Of course, I imagine #3 can easily activate a dormant  
#2 and produce #1.
